Cramps or a bigger problem?

I have been getting UTIs on and off for about a month or so. Just this afternoon I started getting waves of side/back pain. It’s like an internal achy pain. It doesn’t feel like my usual cramping but I am due for my period.

You need to start with a urinalysis and culture. You also need a renal bladder ultrasound to see if you have any stones causing obstruction or another anatomical cause that is giving you UTIs. If all the above is negative you would need a cystoscopy to make sure you dont have interstitial cystitis. Endometriosis can do this as well so a visit to your GYN would be prudent if your urologic workup was negative.
