Kidney working only 14% is an acute problem or a chronic problem. Also, why are you on antibiotics? Short course or prolonged antibiotic therapy. Seems like you have stage 5 kidney disease and you should be under the care of a nephrologist at this time. Depending on your conditions, the nephrologist will let you know how to proceed from here. Ciprofloxin can itself can cause kidney disease. Ciprofloxin and medicines like Ciprofloxin need to be used cautiously in the elderly and in patients with kidney disease. Dose of Ciprofloxin also needs to be adjusted in patients with known kidney disease.
You are either at stage 5 of kidney disease, which means advanced disease close to needing dialysis. Other possibility is that this is an acute allergic reaction to ciproflixacin due to allergic interstitial nephritis. It will need to be assessed by your exam, blood and urine evaluations. You must see a nephrologist asap.

Ciprofloxacin can be harmful to the kidneys as it can cause crystals in the kidneys and may worsen renal function. It should be used with extreme caution in the elderly with kidney disease and the dose should be adjusted to the level of kidney function or an alternative medicine can be used instead. The prognosis with 14% kidney function depends on a lot of factors including comorbidities and if there is symptoms from renal failure.
1st off, I would try to get your doctor to do a 24 hr urine for creatinine clearance. You MIGHT have better than 14% kidney function if that number is based on the eGFR. If in fact your % function is 14% then you may have increased fatigue, mild nausea and generally don't feel great. Depending upon the cause of your decreased kidney function it may continue to worsen over the next 3-12 mos and you may need to start dialysis, depending upon your doctor's recommendation. Avoid all meds hard on your kidneys particularly over the counter meds such as ibuprofen/aleve/advil. Acetaminophen is ok.
Kidney function at 14% of normal in a very elderly patient - I'd focus on excluding easily reversible causes (obstruction, infection etc) and then my main focus of care would be on symptom control (treat anaemia with erthyropoeitin if indicated), ensure not fluid overloaded, and establish her goals and wishes vis a vis end of life care. Generally I would not recommend dialysis in this age group as unlikely to prolong life but could adversely affect quality of life. I would be interested in rate of change of function. I would ensure kidney toxic drugs avoid (especially NSAIDs), advise regarding fluid status and be aware that an intercurrent illness (e.g. a cardiac event) likely to lead to end stage kidney failure. Long term prognosis guarded but can be meaningful if remains well.
Stage 5 kidney failure. Strong possibility that this patient will need dialysis relatively soon. Cipro should be adjusted to once daily dosing.
