What to Expect

Transition requires group effort from family, the pediatric staff, and from everyone else that were involved with past healthcare transactions or services. Some hospitals have a special area wherein young diabetics share the same staff with the adults. This method could be effective in helping adolescents slowly and smoothly transition into an adult diabetic care provider.

The clinic and people that were involved with looking after the adolescents with diabetes should have an established and open communication with the new health providers. The reason behind this is so that they would be able to exchange crucial information related to the patient’s health history and talk about possible issues that might possibly occur in the future.
