Annual check up with new doctor?

I was diagnosed with constipation and a small bowel blockage when I was about 16 or 17 and before I left the doctor's office I was given a digital rectal exam. And I also take concerta 36 MG but sometimes when I take it, I get me light headaches throughout the day. And now I am 20 years old and I am seeing a new doctor next week for an annual check-up because I haven't been to the doctors in 2 years. When I see my new doctor should I ask for a rectal exam because I know I'm supposed to get a rectal exam every year?

A rectal examination at this age is not required unless there is an ongoing anorectal problem.
Your doctor should know what exams you need when you present to her with your symptoms and your prior medical history.
Based on what you have described, I don't see any reason that you need a yearly rectal exam. Perhaps in the case of a defecation disorder contributing to constipation, it might help to do the rectal exam, but generally speaking, a yearly exam is rarely indicated. Just let your new doctor decide if you need the rectal exam. I would bet that it is not needed.
There is no indication for you to have an annual digital rectal exam based upon what you have presented.
