The answer is yes, a previously treated tooth can be infected again due to a lot of reasons. If you feel your tooth is infected again, talk to your dentist immediately.

The key for a successful root canal treatment beyond the obvious, of a well done procedure is having the tooth properly restored with a crown. The biggest reason for failure of a root canal is failure to restore the tooth. This results in potential for infection, fracture, etc.
Most definitely! Even if the root canal is done perfectly, & a crown is placed immediately after finishing the root canal to seal the tooth & prevent bacteria getting inside the nerve space, an infection can still happen. However, it is worthy to note root canals are an excellent treatment to save the tooth, and are successful about 90% of the time.
