Gastroenterologist Questions Celiac Testing

Celiac Testing

I experience severe gastrointestinal symptoms like diarrhea, gas, bloating, and cramping after gluten intake - even very small amounts (1oz.). Symptoms often appear right after gluten intake or sometimes the day after and it can take over a week for all symptoms to go away. Now off gluten and feeling much better. I have done lab work (tTG and DGP) to test for celiac while still on gluten and testing was normal. Can it be a false negative? Should I have an endoscopy done, is it more accurate?

Good question! It's unlikely that the TTG is falsely negative, but you do need to be on gluten for at least a couple of weeks before testing. There is also a rare situation in which TTG is falsely negative because the patient has an IgA deficiency (which we can also test for).
The more common explanation for your situation is that you are having GI symptoms secondary to fructans, amylse trypsin inhibitors or other carbohydrates that are also found in wheat. When you eliminate "gluten" from your diet, then you also eliminate the fructans and other components that might cause symptoms, Most people are sensitive to these other components, but not so much to gluten itself.
