I am a 38 year old female. I want to know if plastic surgery changes your face?

Yes and no. That depends on the techniques used. Some surgeries change more than others. Rhinoplasty with changes on the hump, lifting of the tip and narrowing can change dramatically the face. Some facial implants and thinning out a heavy neck can also change the face dramatically. Facial skeletal work can also change the appearance of the face. If you want minor changes, it will also give you minor improvements.
