How painful is frozen shoulder manipulation?

I am a 29 year old male. I want to know how painful is frozen shoulder manipulation?

It is a good question. First off, It should never be extremely painful. It may hurt a little, but the pain should go away.

It will also vary depending on how severe the diagnosis is. If it is severe, then we work on gently manipulating it, if it less severe we work on being more aggressive.

Hello and thanks for your question. Pain from frozen shoulder can vary from little pain to a great amount of pain. Often there may be other conditions occurring at the same time such as tendinitis or bursitis. The combination can be quite painful. The good news is that frozen shoulder can be treated successfully.
From my experience, the pain can range from 0-5/10. I have usually noticed less pain when the range of motion increases more during and after procedure.
Frozen shoulder can be very painful to treat. It's important to identify the underlying cause of frozen shoulder, and the treatments may vary depending on if it is primary frozen shoulder or secondary frozen shoulder.
The most common cause for frozen shoulder is a tight subscapularis muscle. The most effective way to fix that is by pressing onto the front side of your shoulder blade, which is accessed with you laying on your back and the therapist pressing under your armpit into this muscle. Because the muscle is too tight it hurts. Work with your therapist / chiropractor and let them know when the pressure is too much. Exhale while they are pressing and focus on something else like your breath or a happy memory while being in communication with your chiropractor about your pain level. You can get through it without a problem and you will be happy with the results when you can raise your arm higher and or reach behind your back more easily. 
The answer to this question depends on what is the cause of the frozen shoulder. That depends on your pain tolerance and severity of your symptoms. Some Chiropractors perform manipulation under anesthesia (MUA) also, which may be an option worth exploring, if your pain is intolerable with even slight movements.
Depending on which phase of frozen shoulder you are in will determine the amount of pain. Phase 1 is usually very painful. Phase 2 can be painful. Phase 3 is the least painful.
Shoulder extremity manipulation done properly should not be extremely painful
It is done under anesthesia and little or no pain.

I have worked successfully on many frozen shoulders. I don't do hard, forceful and panful manipulation. I work with quickness and finesse. I try to minimize pain. The amount of success depends on the cause of the condition, how long it has been there, how contentious the individual is with following advice and type of previous treatment. I usually recommend a combination of dynamic range of motion and contrast therapy which is Ice followed by wet heat (wet towel, shower or jacuzzi). Chiropractic, Physical Therapy and Massage are helpful.
