General plus regional block. Usually both together
The most common way is to have general anesthesia with an interscalene regional block. You can have the interscalene block by itself without any other meds if desired. Or straight general anesthesia without a block.
General anesthesia */- interscalene nerve block
You have different options. If you are otherwise healthy, one common approach is a pre-operative interscalene nerve block for post operative pain with a general anesthetic for the procedure. This approach would minimize the amount of narcotic and other medications needed during surgery and allow for same day discharge. Discuss your options with your anesthesia providers to determine what’s would be best for you. Good luck!
A general anesthetic is used for shoulder replacements.  In addition, some anesthesiologist use a regional block (intrascalene block)  to assist in postoperative pain.
