What are the factors considered before a kidney transplant?

My nephew needs to undergo a kidney transplant. What are the various factors to consider before going in for a kidney transplant? What makes someone eligible?

Things we look at to clear someone for a transplant include their cardiovascular health, BMI <30 and psychosocial factors like compliance with medication regimen. Post-transplant, people need to adhere to their regimen to keep their transplanted kidneys functional.
How bad the kidneys are, any contraindications for surgery, what the cause of the kidney problem is, if she's followed by a kidney specialist, etc.
The blood type of the donor has to match the patient and the donor should not have Diabetes or high bp

Tissue matching
Recipient undergoes lots of tests to make sure he is a good candidate, including his cardiac status.
