“How do you qualify for home health care?”
My grandmother wants to be taken care of at home because she's been at the hospital for so long. She's really sick. How does one qualify to have home health care?

If you think she is really sick, that she is spending a significant part of her time in hospitals and she wants to be comfortable in the company of loved ones at home, then you may want to consider hospice care. Hospice usually would be able to provide significantly more services than home health.
She may qualify for hospice at home if she is very ill or she can have home services and this should be discussed with the hospital social worker.
