What causes eyes to not focus together?

I am a 23 year old male. I want to know what causes eyes to not focus together?

There is not only a single cause. You need to see an ophthalmologist.
Eyes “not focusing together” can mean many different things. If the two eyes are not aligned straight together, this is called strabismus and can require prism glasses or eye muscle surgery to fix. If the two eyes have very different glasses prescriptions or powers, this is called anisometropia and needs an eyeglasses exam with a skilled optometrist or ophthalmologist to find the right prescription for you. If the difficulty is focusing at near due to age (usually after age 45 or so) this is called presbyopia and requires reading glasses or bifocal glasses to fix.

It can be you have 2 different prescriptions and the brain cannot focus them simultaneously or there is an eye alignment issue or an accommodation issue. It is best to get it checked out.
There are several reasons for the eyes not to track together, such as underlying strabismus, cranial nerve palsies and other neurologic problems, refractive errors, etc. May I suggest you discuss your particular situation with an ophthalmologist.
Most causes are muscle imbalance. Also large prescription differences between your eyes
Squints may be a cause, see an eye surgeon for review
Binocular vision problems, accommodative problems, eye muscle problems.
