I am a 28 year old male. I have pain in my spermatic cord. What causes pain in spermatic cord?

Spermatocele. Most likely. If you want to have kids one day, don't panic if its one side. To diagnose it you need an ultrasound. If you exercise please wear supportive compression shorts. Under Amour is my favorite brand. Some of mine have lasted 20 years! Air dry only.
Pain in the spermatic cord may be caused by infection, trauma, over exertion and even varicose veins in the area. Since torsion of testes can lead to pain and death and gangrene, any undiagnosed pain in this region requires immediate evaluation by a doctor and ultrasound evaluation. Dr. Zoraida Navarro, MD
Inflammation, Trauma and infection are all reasons for pain in any organ including the spermatic cord. Urologists may be able to help you with the diagnosis and management as appropriate. You can see your PCP if you need a referral.
It could be the result of some infection, need to be tested and treated. Also, in some cases, if it's too painful, then it is an emergency and needs to be seen by ultrasound, and if there's a problem, you need further intervention.
