Why is the first day of my period so painful?

I am a 23 year old female. I want to know why is the first day of my period so painful?

Sudden release of important hormones
During your menstrual period, your uterus contracts to help expel its lining. Hormone like substances (prostaglandins) involved in pain and inflammation trigger the uterine muscle contractions. Higher levels of prostaglandins are associated with more-severe menstrual cramps which is usually the first 1-2 days of your period and sometimes starts before your period.
Stay Healthy and safe!
The pain can be related to the amount of bleeding that you have during the first day. Most of the time, ibuprofen can aid or birth control can aid.

Painful periods are quite common in women, and might be due to uterine contractions (primary dysmenorrhea) or from other reasons such as endometriosis, adenomyosis. First line treatment is over the counter non steroidal antiinflammatory such as ibuprofen (Motrin, Advil, etc) or naproxen (Aleve) as well as regular aerobic type exercises and healthy diet. Birth control pills or other hormone contraception would be the next treatment recommendation. Surgery is reserved for cases that are not helped with medication, or if pregnancy is desired.
Don't absolutely know why by if MOtrin or Aleve don't help you can try Birth control.
