Distressed and defeated
You may have developed a yeast infection after 3 courses of antibiotics. Try over the counter Monistat (7 day or 3 day) or ask your provider for Diflucan. Has your sexual partner also been treated? Dr. Joyce READ MORE
Birth control and period
This kind of pattern can happen if you miss a pill. Try to take your pill at the same time each day so you get in the habit, like when you brush your teeth before bed. If you can be very regular with taking it, the cycle will become regular. If you're worried about pregnancy, then do a home test. READ MORE
Missed period
I would suggest doing a pregnancy test to be sure. It's not likely menopause. It's quite common to skip periods when on long-term birth control. If you prefer to see a period come monthly, you may want to discuss a change of pill with your doctor. READ MORE

No. There are about 30 different strains. If you’ve been vaccinated, you are protected from 9 strains. You can get a different strain from a partner. READ MORE
Birth control and period
It’s normal to bleed when you miss a pill late in the cycle. If you’re worried about pregnancy, then do a test, but it’s unlikely in this scenario. However, if you miss a pill or 2 early in your pack, it can allow you to ovulate and become pregnant, so back up is advisable if you miss more than one pill. READ MORE
What do I do with my daughter vagina?
Vulvar itching can be due to many different things- the fist thing I would want to know is if there are any potential irritants or allergy- causing products that are coming in contact with her vulvar skin- bath bombs, soaps, body wash, lotion or new fabric softeners, etc. if it seems like it could be bug bites, does she have them elsewhere? Does any other family member have similar skin findings? In adult women itching can be from a vaginal yeast infection- this is less likely in an 11 year old. I would have her see her primary doc or pediatrician. Stop all potential irritants and you can try cool baths, cold packs to calm the itching and you can apply some hydrocortisone cream to suppress the inflammation. READ MORE

Bleeding a week after period
On a progesterone-only pill, irregular bleeding is common; it can be due to stress due to your illness or due to the steroids (prednisone). It also can be due to an ovarian cyst, or infection. If it doesn't resolve, I would see your GYN. READ MORE
Paranoid
This sounds a little like obsessive-compulsive thinking. I don't think you need to be worried about contracting or transmitting an STD in this setting. But you may want to talk to a counselor about OCD. READ MORE
Abnormal Sensation
You may have a condition called persistent genital arousal disorder. Medications, stress, and certain types of exercise can be associated, but it is not well understood. Do some research yourself and connect with gynecologist and a therapist with an interest in sexual medicine. READ MORE

Adenomyosis Solution
Myomectomy is a surgical option ; removing adenomyomas is a little tricky technically because they don't always have distinct capsules. Surgery decision would be based on location and also size of the fibroid/adenomyoma(s). Birth control pills are definitely worth a try. You could also ask if Myfembree is an option for you (approved for up to 2 years for women with heavy periods due to fibroids, and also indicated for endometriosis). READ MORE

Pregnancy
Taken properly, combined oral contraceptives are 99% effective at preventing pregnancy. If you’ve missed any pills, it could be less effective. This sounds like a primary herpes outbreak. Go to urgent care and get a prescription for an antiviral medication. You can also use topical lidocaine gel to calm the pain/itching, and take Motrin/Advil. You may also want to get a panel of STD tests. Dr. Joyce READ MORE

When Dr. Joyce was young, she wanted to become a veterinarian. But as it turned out, she was allergic to animal hair, hay, and grass. During high school, she shadowed an OB/GYN in her local community in Boulder, Colorado, and that clinched her destiny.
Obstetrics and gynecology (OB-GYN) is the medical specialty that encompasses the two subspecialties of obstetrics (covering pregnancy, childbirth, and the postpartum period) and gynecology (covering the health of the female reproductive system – vagina, uterus, ovaries, and breasts). An obstetrician-gynecologist, or OB-GYN, is a healthcare professional who specializes in female reproductive health.
In 1984, Dr. Joyce graduated with her Bachelor of Arts degree from Mount Holyoke College. She spent the next year in Fairfax, Virginia working at The Genetics and IVF Institute, where she honed her skill in ultrasound. She then studied at The Johns Hopkins University School of Medicine, graduating with her medical degree in 1989. She completed her internship and residency in obstetrics and gynecology at the Kaiser Permanente Medical Center in San Francisco and then spent six years as a staff physician in Kaiser’s Ob/Gyn Department before joining Women’s Care Medical Group in 1999.
Providing care to women of all ages, she is board-certified in obstetrics and gynecology by the American Board of Obstetrics and Gynecology (ABOG). The ABOG is a non-profit organization that provides board certification for practicing obstetricians and gynecologists in the United States and Canada.
Trained as a life coach, Dr. Joyce coaches other physicians who may be struggling with career and life stressors. This includes burnout, desire for career pivot, relationship issues, empty nest, as well as issues particular to menopause- brain fog, weight gain, anxiety and depression.
“Through coaching, therapy and some deep soul-searching, I gave up obstetrics and the crazy hours that go with it, to work part-time doing gynecologic procedures, infertility, and well-woman care. I love coaching my patients on wellness strategies, and stress and lifestyle management, in addition to their medical conditions, especially perimenopause and menopause. Through my expertise in women’s issues and in-depth, specific training in physician coaching, I also developed my own coaching program for women physicians who may be struggling” stated Dr. Joyce.
As part of her burnout journey, she became a HeartMath Certified trainer. Through breathing practice and mindset, this particular set of techniques helps her manage stress and the negative effects of cortisol on the body. Unlike meditation, these techniques can be used in the moment when stress hits. They can give an individual a sense of “alert calm,” and open the intuitive and creative channels of the mind. With regular practice, one will be able to de-stress more quickly, sleep better, be more creative, and enjoy life with a more balanced attitude.
Dr. Joyce’s professional affiliations include the American College of Obstetricians and Gynecologists (Fellow), the American Medical Association, the North American Menopause Society, the American Medical Women’s Association, the California Medical Association, and the American Association of Gynecologic Laparoscopists.
Happily married with two young adult children, Dr. Joyce is also an avid golfer. She is co-chair of the Sequoia Hospital Foundation’s annual Golf Tournament, and is a several-time champion of her golf club’s women’s group.
