    Diaphragm works well in combination with spermicide gel, and if used correctly can be very effective. Many young women choose to avoid BCPs and use condoms. If not used correctly it will not be effective - best to have an appt with your GYN doc for a fitting and education on how to use it. READ MORE

    The epidural and is a patient choice. I prefer that labor is confirmed so ideally 4 cm the earliest to get an epidural, but when there are other signs like ruptured membranes I allow it even at 2-3 cm if the head is low, again if the patient requests it. Sometimes it is useful if the mother cannot relax and dilatation progressed slowly - the epidural can help. Many women prefer natural births and use IV medication like stadol or fentanyl as needed.     It depends on your age, but there could some serious issues that can cause pain that could be treated early to avoid consequences. Please make an appointment with your GYN doc.     Yes if you are cleared by your OB doc you should be safe to fly. Keep in mind airlines tend to get nervous and need a clearance letter, and some airlines do not let you fly if over 28 weeks especially on very long flights due to the risk of labor. READ MORE

    The standard of care is to start pap smear at age 21 - please remind your doc about this - also we can now check STI tests in sexually active young women like you, under 21, by testing the urine so there is no need for a pelvic exam.     The placenta usually moves away from the opening of the uterus as the uterus grows and the pregnancy progresses- in the meantime avoid anything that can cause contractions as they are the cause the bleeding happens. Avoid intercourse and lifting heavy weights, bending repeatedly or becoming dehydrated as well as any serious exercise other than gentle waking. Have hope! This pill is not designed to be taken every time you have sex, especially if you are regularly active sexually - please be aware it could disturb your cycle and confuse your body if you use it regularly - 1-3 times a year or so is ok - please consider regular birth control which is safer if you need to use it more often then that.     Young girls get exposed early nowadays to different chemicals contained in various plastic containers - one of them is called BPA, a chemical that mimics the estrogen hormone - I would have her hormone levels checked and look into limiting or completely eliminating all BPA containing water bottles, soft drinks, plastic containers. Also never microwave food in a plastic container, only glass. There are some medical conditions that can lead to an early period - a doctor can easily rule them out.     Consider getting tested for the BRCA gene, or maybe your mother can get tested and then if she is positive you should seek genetic counseling. You can get a baseline mammo at 35, but consider annual breast exams, and annual breast ultrasounds as well as possibly thermograms at any age. There is no study to show that exposing your breasts to digital radiation year after year increases your risks of cancer, but more and more women are worried about this issue. Look into checking your hormones, and how you metabolize your estrogen. READ MORE

    Try to avoid going into the scanner - we do not know the effects it can have on a developing fetus and there is no study to show it is safe for babies - please opt out. READ MORE

  • How can I differentiate between implantation bleeding and periods?

    If your period feels off in any way - a few days early or a few days late, maybe lighter or shorter than you expected it to be or maybe more "crampy" than usual then it is possibly an implantation bleed, especially if you start feeling tired and have breast tenderness and get hungry and nauseated at the same time. Pregnancy tests are so sensitive nowadays they can sense the HCG hormone in the urine a few days after implantation. READ MORE

    Baking soda, boric acid suppositories you can buy on amazon (not to be used during vaginal bleeding), also avoid soap on the area and avoid any carbs in your diet - they all take time to work - you get much faster relief with MONISTAT over the counter, or prescription Terazol cream or oral Dilfucan.     UTI manifests as burning when you pee, pain in your lower abdomen, maybe bad smelling urine or even blood in the urine or a strange urine color.     Please consult a doctor - it could be an ovarian cyst developing and growing. An exam and maybe an ultrasound in the office can easily point to the cause of the problem. READ MORE

  • Is it periods or implantation bleeding?

    It is possible it is implantation bleed, but it is also possible you had a longer cycle than usual and you just ovulated later than usual (maybe due to stress). READ MORE

  • Is a TSH value of 3.17 normal in 9 months of pregnancy?

    That depends on whether you are taking thyroid replacement or not. I tend to look at levels of free T3 and free T4 as being more important than TSH since TSH is made by the pituitary and it is not a thyroid hormone - I like my hypothyroid patients to have a TSH between 0.5 and 1.5 in general so yes the TSH might be too high indicating need for more thyroid hormone. I hope this helps - talk to your doc about checking thyroid levels. READ MORE

  • Why am I bleeding after my pap?

    When we do a pap smear we use a plastic spatula that gently scrapes the surface of the cervix to collect the cells - it is not unusual to bleed a little afterwards, however if it happens every time you have a pap discuss it with the doctor and she/he can evaluate you for a possible vaginal infection, ectropion (inner mucosa of the cervix is spread on the outside more than usual). We do pap smears every year so if you need a pap every 6 months there might already be some issues with your cervix that cause the bleeding with the pap. If you have abnormal pap smears you might consider vaginal boric acid supp from amazon 2 times a week (not on your period) and start DIM 100-200 mg 2 times a day for 6 months. Of course discuss this with your doctor first. READ MORE

  • How much Colace can I take while pregnant?

    Try other things that can help with constipation like: - drinking 3-4 liters of water daily, - taking prune juice daily in am empty stomach, - drink a smoothie daily with kale, spinach, apple, carrots, and maybe grapes or pears as well (coconut water to mix), - - adding fiber like metamucil or benefiber to your diet (1 tablespoon mixed in 10 oz of coconut water or 1/2 water and 1/2 juice of your choice). - change your prenatal vitamin - walk 1-2 miles daily (20-40 min) Also cut down on meat, breads, pasta and cheese intake.     Cerclage is a procedure done under anesthesia - the uterus entrance is closed with a purse string suture threaded through the cervix- it is to prevent loosing the baby, which can happen if the entrance is weak. The weight of the baby pushes on the weak cervix and it will dilate it, also the water bag is exposed and can easily rupture, leading to miscarriage.     While you take methotrexate it is advisable to avoid pregnancy. You can control RA with natural techniques and some patients were so successful that they lead normal lives, free of pain and with no arthritis issues. Change your diet to avoid any nightshades, eliminate grains and dairy and any sugar (look up autoimmune diet on google). Also consider looking at whether or not you have intestinal permeability (leaky gut) since it has been implicated in causing the autoimmune dysfunction that leads to RA, lupus, Hashimoto's etc READ MORE

  • Is fertility reduced with spina bifida?

    We do not know exactly what causes spina bifida, but we know that it is linked to folic acid and possibly other B vitamin deficiencies. The risk of spina bifida is very low, 1 in 1000 to 1 in 5000, however it is higher if a family member had spina bifida, as high as 1 in 25 (4%). The mother needs to take folic acid for 12 weeks before conception to help prevent spina bifida in the baby. Also testing the homocysteine levels, and if elevated, testing for the MTHFR gene in the mother can tell you whether the mother-to-be needs to take folate instead of folic acid. Good luck! READ MORE

  • Is pregnancy possible after chemotherapy?

    Please be aware that pregnancy planning should be discussed with the oncologist - the cancer doctor. After breast cancer, sometimes pregnancy needs to be avoided for a few years since the elevated hormones of pregnancy might increase the risk of the cancer coming back. If your sister has normal regular periods after chemo, there should be no issues getting pregnant. If her periods are not returning just yet, she should be patient and consider testing of her other hormones like thyroid and adrenal to see if they were affected by the chemo and stress. Both thyroid and adrenal health can affect ovarian hormones. Once pregnant, your sister should have no issues staying pregnant. The chemo affects the ovary but once periods start, everything is back to normal. As far as breast feeding after mastectomy - once the breast tissue is removed, there should be no possibility of breastfeeding from the mastectomy side, but the remaining breast should allow breastfeeding.   • What if I miss a week straight of birth control pills?

    When you stop taking your birth control pills for 2-3 days in a row, the uterine lining will shed, so you can expect a period. Also, you will loose any protection offered by the birth control pill. Options to avoid missing the pill: - The pharmacies have nationwide chains and they can transfer your prescription where you are; - Also, I refill and send prescriptions to out-of-state pharmacies when my patients are in your situation, so call your GYN doc and explain you left on vacation and forgot the pills and can they send a prescription to a pharmacy near you? Most docs have electronic records now (when I send an electronic prescription to any pharmacy, it gets there after I click send). READ MORE

  • Did my dental x-rays harm my baby? (first trimester)

    Almost all dental offices employ digital X ray technology which means less exposure in general. Also the baby is very resistant to minimal insults and it is very unlikely that any harm would occur. There is always a risk of some unquantifiable harm like risk of cancer later in life but there is not much we can do about that at this point.
