  • What happens when you come off antidepressants?

Taking a good history is the key. Getting the proper diagnosis is imperative. I.e. is this a first time depression , is it recurrent ,is it a depression of Bipolar one or two. Is it a depression of secondary disease for example Hypothyroidism etc. is it secondary to a loss of a loved object or any kind off loss , it may surprise it may be due to a promotion. Knowing all the above and the personality of the patient. We advise according to severity to cut the antidepressant slowly and watch if pt. Depression recur we r committed to one year. After that in few month we try to lighten up and the symptoms recur we r committed to two years. Unfortunately, there r long term depression that we tease out with proper history. It’s advisable to have psychotherapy with the antidepressants and this will give us more clues about cutting the antidepressants and it’s more efficient to have psychotherapy with the antidepressants , and it makes good sense. • What does it mean to be a manic depressive?

It’s a psychiatric disease that usually starts with late adolescence and early adulthood. It runs in families and unfortunately from mother’s Side. It’s now accepted as gene that cause the illness. It can be mild to severe with psychotic features. Nowadays in USA It’s called Bipolar one and the milder one Bipolar two. Usually it starts with Mania or Depression and change to depression to mania or otherwise. U may have more depression than mania or otherwise. Some have pure cyclic depression or pure cyclic mania. It gets complicated when it becomes rapid cycling either in depression or mania or mixed. Sometimes it’s difficult to distinguish from schizophrenia specially in the severe forms were hallucinations and delusions r common. In Europe they name manic- depression , depressions of different severity as Bipolar 1 ,2,3 etc until 7. Treatment is challenging when rapid cycling occurs , or over treatment of mania into depression and vice versa. READ MORE

  • My daughter is starting to tell me she is hearing voices. What is causing this?

Hearing voices in a clear state use to be a pathagnomic of Schizophrena. Now we think of this auditory hallucination as a secondary symptoms of Schizophrenia. The most important thing is to have a psychiatrist to exam her to rule out other diagnosis. Treatment is using anti psychotic medication and slowly and eventually they become less troubling and may disappear. READ MORE

  • Are there factors that could put someone at risk of developing bipolar disorder?

Bipolar illness is an inherited illness, but there are causation to make it appear. Increase pressure at work or home. The earliest signs are inability to sleep and all activities speed up. If diagnosed early, it’s a TREATABLE illness and to engage a good psychiatrist. Usually, the pt. will not agree and the family almost always bring the patient to treatment. It’s dangerous because patients use home treatments like alcohol to sleep and slow down and you will have two diseases, and if the patient is at an age to have other illnesses, it will show in organic diseases. Nevertheless, it is TREATABLE if we as psychiatrists see the patient EARLY and will have a good prognosis. READ MORE
