I decided in 2004... Â to do the unheard of.... Starting a outpatient only nephrology practice. Â I walked away from a large hospital based practice and started from scratch a practice that focused on providing quality outpatient care to the " chronic kidney disease" community, hence the practice name, "Center for Kidney Disease and Hypertension", coined before other copycats. Â The practice model included a proactive plan that involved intensive monitoring and tailoring of care to CKD patients. I also maintained an active role in the dialysis units and currently my practice of two physicians has 3 dialysis medical directorships. Â We are closely aligned with FRESENIUS Medical Care. Â Recognizing where medicine is going and the positive impact the practice model has on longterm care, I am currently work on my CPE certification and will specialize in population health management in the CKD/ ESRD patient population.Â

My mother sees Dr Irwin-Scott frequently. It is because of her careful observation and continued care my mother is not yet on dialysis. She was told by another doctor 2 years ago she would need to start at that time. We saw Dr Irwin -Scott for a second opinion and have never left her. Two years later .... No dialysis yet. She is wonderful, very caring and informative.

Dr. Irwin is ultimately the most personable doctor I have been to. She actually cares for you and follows up always! I am so happy that another doctor referred me to her. The girls in the office are also fantastic.
I saw Dr Irwin-Scott as a new patient. She took her time and explained to me what kidney disease means. I was scared when I walked in and felt better when I left. I know she will watch over my kidneys.
