Stress is anything that affects functioning: it may be emotional in cause or physical or a combination of both [such as pain]. The effects of long term stress do include physical changes in the body such as elevated stress hormones, such as cortisol. Any severe and sharp chest pain requires an evaluation in the nearest ER. Also, consult with your primary care provider to arrange a consultation with a cardiologist for you to make absolutely sure that there is no related heart condition before fully attributing any of the pain to your anxiety. Why such things happen are really unique to the person suffering. Distraction techniques like leaving on the TV while you start to fall asleep helps some people. Others can divert the ruminative thinking with a mental task such as counting until that becomes boring enough to fall asleep. You can also consider asking your provider to request a formal sleep study to make sure there is nothing else going on that might affect your sleep cycle. No one is going to say it is absolutely safe to mix alcohol with psychiatric medications of any kind. But Zoloft does not affect the same brain systems that alcohol and some other medications do. So it is likely not absolutely forbidden, at least for some people. That being said, it is the ideal for you to speak directly with your doctor and see what that person's perspective is on the question.
