Grandmother with acute CKD

Not specifically for me but for my grandma. She's 70 and has acute CKD (stage 4) with a history of high blood pressure and is a type 2 diabetic. In the span of a month she went from stage 3 to stage 4 and has been hospitalized many times. She almost had a heart attack and is currently suffering from edema. She doesn't take any medications except Tylenol (refuses to) and her symptoms are only worsening. She's told me she's dying anyway so it doesn't matter what she does. Realistically, how much time does she have? What can I do to make her more comfortable?

Female | 70 years old
Medications: Tylenol
Conditions: Type 2 diabetic, high blood pressure, irregular heartbeat

At stage 4 she does need to be on Dialysis. However, if she has cardiac problems like Heart failure that could be causing her to swell and also can cause rapid worsening of her gfr, if not managed appropriately.
