What causes a ringing in the ears?

I've had a ringing in my ears for years. What's wrong with me? What could cause a ringing in the ears?

Most commonly degenerative cochlear hair cell disease from aging or acoustic trauma. Many other less common causations, i.e. head injury, bruxism, vascular lesions, etc
