when should i call an ambulance for a migraine

the migraine started on 7-13-22, at 7:30pm. i was already at the hospital and they gave me immatrex, the headache went away but came back yesterday at 11pm. and has not gone away since, i have taken excedrin migraine for the pain. once last night and once today. the pain has gotten progressively worse, my current symptoms are severe pulsing pain in the left side and around my eye, sensitivity to light sound and motion, dizzyness/weakness, shaking/trembling, and the most recent symptom is pain radiating down the back of my neck/a tightness in my neck all on the left side. i am home alone and the pain is too severe to drive myself. i take prazosin (1mg), ziprasodone (80mg), effexor (75mg), and lamotrigine (200mg) at what point should i call for help?

It sounds that you have a Complex Neuropsychiatric Syndrome which migraines can be a part of. Talk to your Neurologist or Psychiatrist about reassessing your medications and changing things around a bit to see if you respond better and achieve better control of your migraines. I would particularly discuss with your doctor the new class of migraine medications called CGRP's ( Calcitonin Gene Related Peptides). Many patients find significant relief with little to no side effects for both symptomatic and prophylactic treatment.
IF this is your worse headache, then yes call the ambulance. Of note, this is a the time of year that many people are suffering from migraines.
