Have you seen a urologist for this matter yet? If not, go to see one ASAP because intermittent testicular pain should lead a knowledgeable professional to raise the concern of intermittent testicular twist (or torsion), which is not uncommon. There are many causes of testicular pain, but one must rule out intermittent testicular torsion. It is best to have your son seen by a urologist at the time of pain to eliminate intermittent torsion as the diagnosis. A scrotal ultrasound is helpful in making the diagnosis.

If he is wearing boxers and not supportive underwear, this happens quite frequently. Have him wear tidy Whiteys or boxer briefs.

He would need to be examined and questioned carefully about when it hurts and what preceded the pain and what he can do to make it better. He could have intermittent torsion of the testicle. but he would need an ultrasound of the testicles and be examined.
Sometimes testicular pain that comes and goes can be from partial twisting and untwisting of the testicle. This can be a serious condition. I recommend review by a pediatric surgeon or pediatric urologist.
