Cardiovascular Outcomes in Type 2 Diabetes
It is known that strokes and heart attacks occur 2 to 4 times as frequently in diabetics than in non diabetics. An article that appeared in an August 8, 2018 edition of the New England Journal of Medicine reviewed the outcomes of 271,000 diabetics in the Swedish National Diabetes Register. I compared...
What OTC pain reliever to take?
Dear Patient, I would use Tylenol 325mg for several doses; if still in pain, then try 500mg tabs to max dose of 3000 to 4000mg daily. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
Possible TED?
Dear patient, Thyroid eye disease is an autoimmune reaction. Can occur at any function level. Need antibody test. See your physician quickly. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
What medicine can he take?
Dear patient, I do not like over-the-counter meds as they can have multiple side effects which may be additive to levothyroxine. I like Tamiflu for flu, nose drops for two to three days, and acetaminophen for aches and pain. He should consult his doctor for accurate diagnosis. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
How do you manage diabetes at work?
Dear patient; you manage diabetes at work as at home, no changes to regimen. Bring all meds and supplies to wOrk and use restrooms to administer injections and other meds as needed. Respectfully Marvin A Leder MD FACP FACE READ MORE

I have covid stomach pain?
Dear Patient: Prilosec can cause diarrhea; try Zantac. Speak to your physician as stopping Clarithromycin may help as it can cause multiple gastrointestinal problems. and is usually used for respiratory illness. Respectfully Marvin A Leder MD FACP FACE READ MORE
Does alcohol use lead to diabetes?
Dear Patient, Severe alcohol use leading to pancreatitis can cause diabetes, requiring treatment. In type 1 diabetics where alcohol intake was studied, sugars tend to be low and may lead to severe hypoglycemia due to alcohol's effect on metabolic cycles, which control sugar balance to protect us. You have an fbs of 105, which indicates pre-diabetes, but no family history of diabetes. I would have liked to know your A1c level. At this point, I would suggest following up with your physician periodically and keeping alcohol intake to two drinks or less after dinner, protecting against pancreatitis or liver disease. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
Replacing levothyroxine w/ an all natural supplement?
Dessicated Thyroid is natural product which is obtained from animal tissue. It contains raw thyroid elements. It has held up well on some clinical trials. I prefer synthetic preps such as Levothroxine because I know what is in them, their onset of action is quicker, however, if you wish to try them, go to it, but monitor her feelings and blood work, tsh. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
What causes yellow but jelly-gel on your bottom?
Is it stool? If not stool, I don't know know and would suggest seeing a dermatologist. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
Do I have high blood sugar?
Dear Patient, You do have very high sugars and should immediately be seen by your physician to prevent serious complications from occurring. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
Are my progesterone levels ok?
Dear Patient, If you have a usual 28-day cycle, it seems like progesterone level is low. The progesterone levels surge with ovulation which usually occurs on day 14. If your egg is not fertilized, the levels start to fall at about 10 days after ovulation. I am uncertain if you had an ovulatory cycle. This should be discussed with your gynecologist or a reproductive endocrinologist. The past hypothyroidism, which I assume is now controlled without meds, is not a problem. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
Do I have a parasite?
Dear patient, Tell the physician your symptoms and ask for stool tests. Respectfully, Marvin A. Leder, MD GACP FACE READ MORE
Do I have anemia vs fibromyalgia?
Dear Patient, There are many biomarkers of iron deficiency anemia; also if placed on iron therapy and your symptoms improve then that supports diagnosis of iron deficiency. Fibromyalgia presents with certain areas of body discomfort and normal laboratory work, which would rule out iron deficiency. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
Are Hypokalaemia and Hypoglycaemia related?
Dear patient, Complex question; first, adrenal insufficiency causes high potassium because of lack of hormones excreting potassium. Cushings disease and hyperaldosteronism lower potassium. Excess insulin due to overeating carbs will move potassium into cells, causing low potassium and in severe bulimia when blood volume low potassium is lost due increased aldosterone excretion by adrenal glands. In severe insulin-induced hypoglycemia, a low potassium would reflect intracellular shifts of potassium. You don't mentIon eating licorice, which can also cause low potassium. I would suggest you speak to your physician and possibly be referred to an endocrinologist and nephrologist. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
How to treat a hematoma on my leg?
Dear patient: ice for 15 minutes every few hours, but see your physician to be sure you only have a hematoma. Respectfully Marvin A Leder MD FACP FACE READ MORE
Diabetes 2?
Dear patient, You did not state A1c in March, but had great response to treatment with Metformin. I don’t doubt diagnosis but keep up good work with diet, metformin and make sure your blood pressure and cholesterol are also at goal. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
I'm not sure if I am diabetic or not?
Dear patient, An A1c of 6.5 or above establishes diagnosis of diabetes. You have had several a1cs above 6.5. Pre-diabetes is diagnosed with A1c of 5.7 to 6.4. You do not mention your blood sugars. The drug Crestor lowers cholesterol not blood pressure. I am not familiar with the drug Lomax (please recheck name of drug). I would suggest you speak to your physician about treatment with anti diabetic drugs to improve control of diabetes in view of elevated A1cs. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
What causes a dull abdominal ache?
Dear Patient, I am sorry for your distress; hypothyroidism does not cause abdominal distress; some women get menstrual cramps with each period. Your abdominal pain occurring during stressful school period could be an irritable bowel. I would suggest you discuss this with your physician and perhaps visit a gastroenterologist for further evaluation and treatment. Most likely there is no serious issue present, but you should learn to cope with anxiety, which is probably fueling your problems. Respectfully, Marvin A. Leder, MD FACP FACE READ MORE
