What can I do to remove the plaque on my teeth? Does toothpaste remove plaque? Or should I visit a dentist?

You should do both. A daily routine of brushing your teeth with toothpaste helps remove soft and easy-to-remove plaque. Hardened/calcified plaque (a.k.a. calculus) needs to be removed by professionals such as your dentist or hygienist.
No, toothpaste traditionally only carries fluoride or whitening. Plaque can only be removed by the mechanical action of brushing. If you have calculus build up (petrified plaque), it will need to be professionally removed.
