Is surgery necessary for anal fissures?

I am a 33 year old male. I have anal fissures. Is surgery necessary for anal fissures?

Sometimes, yes.
Short answer is no. First line treatment is softening of stool and avoiding straining; some patients respond to topical medications. Best to see a colorectal surgeon or a general surgeon in your area that does anorectal surgery to determine if you need surgery.
Not necessarily. They can be treated with nifedipine cream and see if they heal. Step two might be Botox injection. If these treatments fail and you continue to have pain and have had enough, them surgery may be warranted.
