Another possibility is the preventive treatment with Topamax can also cause numbness or tingling ( a combination of both is also a possibility).  The concerning issues are if you have such ( complicated migraines,  triptans = sumatriptan are relatively contraindicated ) while the CGRP's are a new class of both symptomatic treatment ( Emgality ) also can be used as preventive ( Nurtec ). If the Topamax is the primary culprit of your numbness then changing to a different category ( to be determined by you and your Neurologist should be discussed ). If your blood pressure is on the high side usually blood pressure medication such as Beta blockers ( propranolol) may offer significant benefits. Contact your Neurologist and ask him about getting an MRI scan and sitting down and discussing above issues.

If you suffer from migraines that are debilitating or incapacitating,  on average of once or more times per week, then you should consider prophylactic ( preventative ) therapy. Migraines throughout childhood into young and nature to middle-aged adults often lead to multiple microinfarcts to the brain. This in turn can lead to significant cognitive impairment later on. On ni so rare occasions in younger people, it can cause clinically significant Transient Ischemic Attacks ( TIA's ) or Strokes with residual sequelae. So to simplify the answer to your question: The answer is a resounding YES.

I would call your primary or a neurologist to get you in and do further testing (nerve studies and or more site-specific MRI or CT scan studies). You might need further treatment depending on what is found. For the time being, I suggest refraining from major strenuous physical activity pertaining to your upper extremities.
