What could be the issue?

My periods are very irregular. My last period was July 1st. For the past week, I've had itchy nipples and been very nauseous. October 4th I had some strange tissue with mucus coming out of my vagina after very light spotting. My mother thinks this is a mucus plug. Every at-home test I've taken has been negative. If it's not pregnancy, what else could the issue be?

What I see in the picture is normal mucus discharge, I would not worry about this. Cycle irregularity and itchy nipples may not be related. However, nausea could be related due to a delayed cycle. How to manage her cycle irregularity depends on whether you want to get pregnant or not. I would recommend visiting your gynecologist and getting evaluated for cycle irregularity.
There are a number of possibilities, but offhand, I’d suggest that you did not ovulate and therefore the menses haven’t resumed. Get script for a 10-day course of Provera.
You should seek evaluation with your provider for evaluation of vaginal discharge, since your pregnancy test is negative it is not likely that you are pregnant but other testing would likely determine the cause of the discharge.
Sounds like your periods are not regular. It could have build up of the lining during those several months. You might consider Birth control if you are not trying for pregnancy. I would be a good thought just to have a doctor visit to see why your cycles are irregular. You might want to check lab like thyroid studies, & prolactin.
