Yes, check out my page https://www.lasvegasbariatrics.com/.
Technically, the NIH recommends bariatric surgery for this with a BMI of 40 or above, or above 35 with an obesity related medical problem. With a BMI of 39, you would also need to have acid reflux, high blood pressure, diabetes or pre-diabetes, polycystic ovarian syndrome, sleep apnea or the like. Your BMI is 39.1 You are strongly recommended to participate in a weight reduction program. Obesity is considered a BMI 30 or over. You are at high risk for many morbid illnesses. Your overall health will improve with weight reduction. Bariatric surgery is an effective way of maintaining weight control. Remember however, preoperative consulting is a part of the program and it will require a lifestyle change.
I am a bariatric surgeon. Yes, you would be eligible. Insurance may or may not cover based on your coverage and the insurance companies pre-set requirements. The cut off is a BMI of 35, usually with 1-2 co-morbid conditions (hypertension, diabetes, sleep apnea, etc). At a BMI of 40 or greater, insurance may or may not require other co-morbid conditions. There are cash pay options if insurance does not cover as well. Make sure you look for an ASMBS designated surgeon. Remember that bariatric surgery isn't magic. It is a tool to help you in your pursuit of health. The surgeon is important, but an actual program with support in the form of dieticians and support groups is absolutely essential if you are to realize life-long success. Good luck on your journey!
Your BMI is 39. With your medical problems, that would make you a candidate for bariatric surgery.

The criteria are BMI >40 or BMI 35-39 with obesity-related medical problems (such as hypertension, high cholesterol, heart disease, sleep apnea, diabetes)
It has to be determined with precision at the time of the medical visit but with a BMI of 39 more likely yes.
Best to check with the facility that would perform that surgery. The MD there knows the benefits and risks depending on your specific needs.
Yes, you also have comorbidities such as, OSA. Please check HgA1c.
